Comprehensive Assessment
A comprehensive, multidisciplinary assessment of fall risk by a Physiotherapist, Occupational Therapist and Registered Nurse to determine appropriate interventions to reduce risk.
Seniors identified as appropriate candidates are referred to the South West Community Care Access Centre for a Comprehensive Fall Assessment.
Required services are determined using the Grey Bruce Falls Prevention and Intervention Program Comprehensive Assessment and a fall prevention plan developed and put in place. A three month follow up is conducted to determine any change in confidence in performing daily activities. Each client keeps a diary of falls.
If 3 out of the 5 below questions are answered yes, senior is referred to South West CCAC for a Comprehensive Fall Assessment: 1. Have you fallen? 2. Have you experienced a near fall? 3. Have you previously reported any falls to a health professional? 4. Have you ever sought medical attention for a fall? 5. Have you ever broken a bone or have you been diagnosed with osteoporosis? If so, are you not currently taking any calcium, vitamin D supplements and/or medications to stimulate bone growth?
