Fall Risk Assessment

 

What is Fall Risk?

 

The factors contributing to fall risk may be intrinsic, extrinsic or both. 

 

Intrinsic factors (those related to the client) may be: age, poor balance, gait abnormalities, decreased vision, cognitive/perceptual factors, fear of falling, previous history of falling, dizziness, and poor strength/endurance. 

 

Extrinsic factors (those related to the client’s environment) may be: poor lighting, slippery floor, uneven floor, lack of hand rails, need for grab bars, clutter, hazardous equipment, and lack of emergency measures.

 

How do I obtain a Fall Risk Assessment?

  1. Complete the 6 point self-screening tool (6 Warning Signs of Falls)

  2. Visit your healthcare provider, local Family Health Team or contact the South West Community Care Access Centre and ask them to complete the Multifactorial Fall Risk Assessment with you.

  3. Contact the South West Community Care Access Centre for a Comprehensive Fall Risk Assessment
