The Grey Bruce Falls Prevention and Intervention Program addresses the needs of seniors who are at various risk levels of falling: low, moderate and high risk, through an integrated system of services within Grey Bruce based on a continuum of evidence-based initiatives.
This comprehensive program consists of 5 components: v 6 point self-screen (6 Warning Signs of Falls) v Multifactorial Fall Risk Assessment v Home Support Exercise Program
The Grey Bruce Falls Prevention and Intervention Program is funded by Ontario’s Aging at Home Strategy. The Aging at Home initiative is designed to allow seniors to live healthy, independent lives in the comfort and dignity of their own homes.
