The Grey Bruce Falls Prevention and Intervention Program addresses the needs of seniors who are at various risk levels of falling: low, moderate and high risk, through an integrated system of services within Grey Bruce based on a continuum of evidence-based initiatives.

 

This comprehensive program consists of 5 components:

v  6 point self-screen (6 Warning Signs of Falls)

v  Multifactorial Fall Risk Assessment

v  Comprehensive Assessment

v  Home Support Exercise Program

v  Community Action Plan
