School Dental Screening

Dear Parent, Guardian or Caretaker,

During a school dental screening, a registered dental hygienist found that your child has a dental condition that requires immediate treatment by a dental provider. The Parent Notification Form (PNF – yellow form) must be completed and returned to Grey Bruce Public Health within 20 business days, as per the mandated dental screening program of the Ontario Ministry of Health - Requirement #5 of the Ontario Public Health Standards.

Instructions for Filling out Forms

Please choose one of the following options when completing the form:

Able to Pay for Dental treatment:

  • Fill out section 1, and have your dentist fill out bottom of section 1

Already have a Healthy Smiles Ontario Card:

  • Fill out section 1, and have your dentist fill out bottom of section 1

Unable to pay and wishing to use Healthy Smiles Ontario Program:

  • Fill out sections 2A, 2B, 2C, 2D and 2E
